Funded partners are required to submit a Work Plan. In this Work Plan, you'll be asked to identify your project objectives, activities, strategy and approach, staff roles/responsibilities, and a timeline that will lead to strengthened programs, sustainability, and increased capacity. You will also be asked to identify your local evaluation plan and communications plan timeline. Please utilize the example template and webinar recording as references when completing your Work Plan.

The Youth Suicide Prevention Media and Outreach Campaign Program team will review your plans and provide feedback.

Local Evaluation Plan
An evaluation plan is a written document that describes how you will monitor and evaluate your program so that you will be able to describe the “What”, the “How”, and the “Why It Matters” for your program and use evaluation results for program improvement and decision making. In this section, include the areas where you will utilize The Center’s evaluation technical assistance.  Use this section to think through the components of your evaluation plan. This plan will focus locally and evaluate only your organization's implementation of the larger YSPP program.
