An evaluation plan is a written document that describes how you will monitor and evaluate your program so that you will be able to describe the “What”, the “How”, and the “Why It Matters” for your program and use evaluation results for program improvement and decision making. In this section, include the areas where you will utilize The Center’s evaluation technical assistance. Use this section to think through the components of your evaluation plan. This plan will focus locally and evaluate only your organization's implementation of the larger YSPP program.

Groups important to project success: Identify the different groups or partners (think about program implementers, those who actively support the program, program participants, key partners and decision-makers). Recognizing their different perspectives and roles in the program, state their importance to the project. (Max 200 words)

Evaluation Questions (up to 2): State the evaluation questions. These are high-level questions that you would like to answer about your funded project. The evaluation questions can be qualitative or quantitative in nature. For example, you might be trying innovative approaches to community mobilization, hence your questions can focus on the pitfalls and successes of the approach or strategies used.
